Requesting Disclosure etc. of Personal Information
How to request notice of the purposes of use of personal information or its disclosure, correction or deletion of content, cessation of use, removal, or cessation of provision to third parties (disclosure etc.)
Submit your request by filling out the Application for Disclosure etc. of Personal Information/Response Form, attaching the personal identification documents below, and mailing it to ADAMS Japan (AEGON Direct & Affinity Marketing Services Co.,Ltd., attn.: Okabe, 28th Floor, New Otani Garden Court, 4-1 Kioi-cho, Chiyoda-ku, Tokyo 102-8578, Japan). A response will be mailed to you at a later date.
Please note that such requests will not be accepted in person at the ADAMS Japan offices.
Personal identification documents:
| When the request is made by the individual concerned |
1) A photocopy of any one of the following: the individual’s driver’s license, passport, health insurance card, or pension booklet |
| When the request is made by a legal representative |
1) Documents that can be used to confirm the right of legal representation,
And
2) A photocopy of any one of the following: the legal representative’s driver’s license, passport, health insurance card, or pension booklet |
Cost of disclosure of personal information:
There is no cost for disclosure of personal information.
